A bit more training is required?
It's called a quality check which a lot of uploaders sacrifice for speed.
Especially if it was broadcasted like that originally from the source.

SP has no quality check as they just upload exactly what was on the service it was ripped from (they do change the font of the dialogue, but that's about it)

To be fair it looks like every team's release of this has the same problem (Apart from one anon release I found)

Most zero day releases are automated. It is madness to suggest that a team that releases something like 40 shows a week in 3 different sizes should manually check every one.

Fansubbing where they're actually translating and coding subs, encoding the video file etc would be quality checked as part of the proceedure
