For those who are interested in what happened: the TS basically started recording too early, and the previous show had mono audio.
[ Mono audio --> Stereo audio (Titan) ]
As a result, when mkvmerge was used to cut the audio, it applied the first set of headers it found (the mono ones) to the outputted audio stream; the data itself is still stereo. Apparently, this was an issue back in the days of Moshidora. As a result, libavcodec devs updated their code to account for this channel count switching error. If the original version didn’t play for you, it means that your codecs are way too old. The v1 will easily play if you update your codecs.

P.S. The patch also contains fixes to a few typos
